Communication and Engagement Reduces Readmission by 20%

Today, the RARE Campaign— for Reducing Avoidable Readmissions Effectively— announced that the hospitals participating in the program have reduced their readmission rates by 20% from their 2009 baseline.

The RARE Campaign focuses on improving communication and engagement in 5 key areas, all of which are areas where DuetHealth offers solutions:

  1. Comprehensive discharge planning
  2. Medication management
  3. Patient and family engagement
  4. Transition care support
  5. Transition communications

The participating hospitals, which together account for more than 80% of all readmissions in Minnesota, have reduced the total number of readmissions by more than 4,000 patients.

As healthcare’s reimbursement model moves from one predicated on the “do more, bill more” model, to one focused on value and outcome, forward-thinking health systems would be well served to follow the RARE campaign’s lead of focusing on improved patient engagement to reduce cost and improve care.
